USA county courthouse - security and emergency procedures

I know that most courthouses these days have metal detectors you need to walk through to enter. Beyond that, what are the security procedures?

Are the guards police, off-duty police, or just security? Are there armed guards in every courtroom with an on-going trial?

If there is an emergency situation (an armed gunman, say, or other type of physical attack in one of the courtrooms) would the entire building be evacuated? Would the guards just maintain some kind of perimeter and wait for the local SWAT equivalent to arrive? Would there be eMTs on hand? What else should I know to keep this within spitting distance of reality?
